Procedure for Admission to
Gratitude House Programs

1. Attend Orientation
2. Set up appointment for initial assessment
3. Obtain all necessary documentation
4. Admission date set by admissions coordinator
5. Admission into appropriate level of care

Documents Required:
 • TB skin test or chest x-ray
• Syphilis test
MITT additional requirements:
 • Proof of pregnancy
• Medical records of infant and mother
• Birth certificate
Hibiscus Haven additional requirements:
 • Proof of HIV status
Dual Diagnosis additional requirements:
 • On-site pre-psychiatric evaluation

Fees for Treatment
Residential Treatment:  
Palm Beach County Resident $350/week
Out-of-County Resident $450/week
Out-of-State $550/week
   
Day Treatment:  
All Clients $200/week
   
Intensive Outpatient Treatment:  
All Clients $100/week

Treatment fees are based on a sliding scale with the entire invoice being the responsibility of the client/family.
